• Bereavement Form - Administrative Staff / Professional Staff

    Bereavement leave may be requested for time off for absences due to the death of an immediate family member or near relative.  Please complete the attached bereavement leave request form and return to the Business Office along with a copy of the obituary to request approval of such leave. 

    Three days or less may be granted for the following:  father, mother,  brother, sister, son, daughter, husband, wife, parent-in-law, brother-in-law, sister-in-law, son-in-law, daughter-in-law or near relative who resides in the same household, or any person with whom the employee has made his/her home.

    One day may be granted  for the following:  first cousin, grandfather, grandmother, aunt, uncle, niece, nephew.
     
    The Board may extend the period of absence with pay at its discretion as the urgency of the case may warrant. 
    Extended bereavement may be requested by submitting a letter to the board and superintendent.  Please include an explanation of the need for this extension and the dates you are requesting.  If the request is not granted you will be notified and any extended absences will be charged as personal or vacation.
